To request travel vaccinations:
- Use our online tool, eConsult – when asked select Adult health problems then Travel advice
Please note that many vaccinations are required many weeks before you travel in order to be effective.
Please ensure that you contact the practice well in advance in order to ensure that we can meet your requirements, especially at busy times. We recommend 8+ weeks before departure.
In advance of your appointment, please also complete the attached assessment form and return to the surgery as soon as possible.

- Please make sure you complete the form fully as failure to do so will result in potential delays to your vaccination schedule.
Occasionally, you may have some urgent reason that means you cannot plan in advance and in these circumstances, it may be more convenient for you to attend a local Travel Clinic. It is never too late to get some protection and we can advise you of other Travel Clinics in the area.
